Change in clinic operations due to Covid-19

We are currently operating via telemedicine and limited in-person visits by appointment. We can be contacted above from 8AM to 3:30PM M-F, and closed for lunch from 12:30PM to 1PM. *Please allow up to 72hrs for a physician call back.

  • All FMLA paperwork, short term disability, Prior authorization forms and the like must be done via in-person appointment.

  • The lab is open and has moved to Suite 102. Please be fasting unless otherwise notified. Lab hours are from M-Th 8:00 to 3PM, Friday is 8:00-2:30PM. Closed 12:30 to 1pm *Last patient must arrive 30 minutes prior to closure of lab

Do not present to the clinic without lab orders, please call the clinic in advance or utilize the Patient Portal to request orders

We are currently out of the under 65 flu shot, with limited supplies of the over 65 remaining.

Faiz M. Uddin, M.D.


Dr. Uddin graduated from Loyola University, Chicago with a BS in Chemistry with honors in 1996. He then matriculated at The Pennsylvania State College School of Medicine and received his Doctor of Medicine Degree in 2000. Dr. Uddin completed his internship at Northwestern University Chicago, and later completed his Residency training at the University of KY Chandler Medical Center with an interest in trauma. Dr. Uddin sees patients of all ages from Pediatrics to Geriatrics with a focus on Adolescent medicine. Dr. Uddin is Board Certified in Family Medicine.

Lung Cancer Screening with LDCT

New School Immunization Requirement Effective July 1, 2018
  • All students in kindergarten through twelfth grade must show proof of having received TWO doses of Hepatitis A vaccine to attend school... Please understand there is a minimum 6 month waiting period between doses. In order to receive the vaccines you must be an existing pediatric patient or schedule an initial appointment to establish care with Dr. Uddin. Further, Dr. Uddin will review your child’s records, health history along with the immunization record to see if your child is even eligible for the vaccines. A physical exam is also required before vaccine administration. This is a multi-step process, so please do not expect to show up get some shots and leave... Please do not panic, as schools allow children to remain in school as long as a waiver is written stating that the process of receiving vaccines has been initiated. Students aged 16 years or older, must show proof of having received TWO doses of Meningococcal ACWY vaccine to attend school. Most children receive their first dose at age 11, with a booster at age 16.
